FCP required to work in Bath across 4 practices. Minera PCN consists of four practices across the west of Bath & we have a collective list size of 37,000 patients & growing. Minerva PCN is a supportive, friendly & forward-thinking PCN. Successful candidate will have access to NHS Pension The post holder will work at an advanced clinical practice level as an autonomous practitioner. A part of this may be day to day managing of the FCP service within the primary care team of a PCN. Post holder will demonstrate outcomes/achievement of key results for the FCP contract & have strong relationships with local clinical specialists within the field of primary care & advanced MSK practice. Scope of the Role:•To be the first point of contact for expert MSK clinical assessment & diagnosis of patients presenting to primary care/GP practices. Be the link between primary, community & acute services, when the patient is managed via primary care ensuring an integrated care pathway for individual patients that meets their needs. This will include provision of triage to other services &/or directly providing management & advice. Lead on & develop effective communication between primary care services/GP practices & other relevant care providers. Educate on the role of the FCP, its impact on referral patterns & patient care pathways. Work with other FCPs & primary care teams to develop collaborative working within the primary care multi-disciplinary team & provide expert advice to support the contribution of FCPs to MSK pathways. To be the first point of clinical contact for patients presenting with MSK conditions or symptoms, providing the patient with the primary assessment. Use professional judgement & advanced clinical reasoning skills to make decisions about safe & effective patient care in unpredictable situations, including when there is incomplete/contradictory information. To be able to work autonomously as a lone practitioner Essential Qualifications: BSc degree in Physiotherapy & registered with the Health & Care Professional Council. Experience: Can demonstrate working at Level 7 [within the ACP framework] capability in MSK related areas of practice or equivalent (such as advanced assessment diagnosis & treatment. : Experience in applying the physiotherapy process to include MSK assessment, interpretation, individual care planning, motivation, monitoring & evaluation of highly specialized MSK treatment, working knowledge of Microsoft & GPIT systems, alongside prescribing data monitoring systems, experience of contributing to service delivery/evolving evidence led practice & delivery in a changing environment, ability to demonstrate leadership when introducing change. Analytical Skills : able to understand & analyse complex issues & balance competing priorities in order to make difficult clinical decisions & support individual patient care by an ability to analyse & interpret complex clinical/ often incomplete information.
