toe extensor tendinopathy

Does anyway have any good ideas for tib ant/ toe extensor tendon pain? its a chronic condition with acute exacerbations, brought on with walking (esp. fast/up hill). pain at highest with heel strike (ie max dorsiflex). I have tried orthotics, cortisone, rest, tib post strength, tib ant eccentric strength, taping of toe extensors, gait re-ed!!! Any thoughts would be appreciated!

re: toe extensor tendinopathy

Have you tried mobilising the ankle or the forefoot - the tib ant/toe extensors may be overworking to compensate for a lack of range in these joints - especailly loaded dorsiflexion of the ankle. Stretching/ soft tissue release of the tib ant region could be worth a try also. Footwear may be worth a look at - are the shoes too big iin the toes or tight across the front? X-ray?? also might be worthwhile to check there is no anterior ankle impingement potentially contributing to symptoms.

Re: toe extensor tendinopathy

i am debating between 3rd metarsul stress fracture and Extensor tendinopath. does the latter feel acutely painful like a fracture? heeling time?
