setting up new sports rehab

I'm qualified physio + fitness instructor currently working in large private gym. We're going to set up sports therapy room, where members can be assessed + have sports massage etc but as we are starting from scratch we need all the advice we can get. Has anyone got any informed consent forms, assesment forms etc that may help us or any other advice or info sources. What insurance do we need? I'm state registered, CSP member + have fitness instructor insurance. most of other staff are gym instructors qualified in sports massage. I hope you can be of help!!
