Research Questionaire

Hi folks,
I am about to carry out a study on the utilization of physiotherapy services by medical practioners in private practice. I need some guidelines on constructing a suitable questionaire.
The study is going to be carried out in Nigeria.

The awarness among medical practionarries is very less as far as i know. It is better to find an answer in a scientific way.... besk of luck...

If you are looking @ utilization are you thinking about how many referrals are generated?
This would be dependent on the nature of the practice. you might want to think about catogorizing the referrals generated - total population that comes to the clinic and are having the diagnosis of interest etc etc
Such as do all patients who require physiotherapy with (eg. carpel tunnel syndrome) that go to a clinic get referred for physiothrapy
I do not know much about Nigeria But how does the funding work? do the patients have to pay from their own pockets or are there insurance coverage? because that will also impact how the referrals are generated
