6 months post- op THR

Good day! I am doing private practice and i have a 72 year old female patient having a PHR last 2 years ago. and last year around November, she was having an operation again due to infection and this time for THR. 2 days after the operation, she was referred for physical therapy. Her treatment were all exercises. So, when the time has come to go home, she still continued her therapy session with her PT. After 5 months, she went to her Hometown, and thats when I take over with her therapy sessions. we were having therapy sessions thrice a week for almost a month now and the treatment were:
1. SLR on affected side (R) from Active Assisted to Active x 15 reps x 5 secs hold
2. Abduction (R) LE from active assisted to active x 15 reps x 5 secs hold
3. Hip flexion, knee flexion (R) from active assisted to active x 15 reps x 5 secs hold
4. Sitting position : AROM on (B) UE in all planes using 10 lbs dumb bell x 30 reps
5. sitting position : hip flexion using thera band x 15 reps (affected side)
6. Sitting position: Knee extension using theraband x 15 reps (affected side)
7. Sitting position: Knee flexion using thera band x 15 reps ( affected side)
8. Reps of No. 4, 5, 6 and 7 on the unaffected side
9. squatting x 15 reps x 10 secs hold
10. launges x 15 reps x 10 secs hold
11. tip toeing x 15 reps x 10 secs hold
12. heel (PF in standing position) x 15 reps x 10 secs hold
13. Full weight bearing on affected side for 1 min. with 30 secs interval
14. bicycle x 5 mins
15. Marching x 15 reps
16. In parallel bars : standing doing AROM hip flexion with knee extended x 15 reps, hip hyperextension x 15 reps and hip abduction x 15 reps ( both LE)
17. Walking

Its already 6 months after her operation and she wanted to ambulate without a quad cane. what other exercise can you suggests to develop more strength and endurance.

by the way, she has slight signs and symptoms of Parkinson's disease like Hand tremors.

i think you need to assess the function
tinneti test (balance and movement, march)
berg test
because in elderly the main outcome is the function

Hi Christine,

It's a terrific program for a “young” old woman. Success is not really related to a large number of exercises but an understood re-education. It is preferable to orient the treatment to functional activities where lower limb is normally solicited in its place (body). 6 months past surgery is a delay where hip is solid and patient has to be free of us!
