AFO contra-indications and precautions

We are starting a pilot program to supply standard AFOs to community based clients.
We are drafting a competency and I was wondering whether anyone had any such paperwork already in place (and was willing to share!)
Or if anyone has any ideas as to what should be generally included excluded outside of ordinary tissue viability issues.

I suggest David Butler's book or Michael Shacklock's recent book on the topic. do a web serch on their names andButler's book is Mobilization of the Nervous System, Shacklock's is just released, (hope I spelled his name correctly, will repost when I find the recent reference to his recent book).
