Interdisciplinary vs Multidisciplinary team work

I am a South African physiotherapist working in a Rehabilitation Unit (neurology, orthopaedics and amputations). Our team currently has a multidisciplinary approach. I am currently changing it, working towards an interdisciplinary approach. It is hard work and very challenging, and takes a while to achieve this. So far we are doing very well. I am very keen to hear what are your experiences out there with these two teamwork approaches.
