The Aged Care Shared Care Program located at Melbourne's northern suburb of Broadmeadows, supports complex aged, community-based clients. You will be providing comprehensive holistic assessment, care-coordination and short-term physiotherapy treatment to elderly clients in their homes. You will be part of a multi-disciplinary team and be provided with a vehicle to use during business hours. This position offers a variety of caseloads including Orthopaedics, Neurology and Respiratory. Previous experience working across a number of departments and with the elderly would be an advantage. This position is available now and duration is negotiable. It is solely a community base position and you need to hold Full Victorian Registration as well as a current and valid drivers licence.
